As a general rule, we will process it according to your request.

2)Procedures for Requests for disclosure, etc.

  1. After receiving the request, we will mail the prescribed request form “Request for Disclosure of Retained Personal Data, etc.” to be used by us.
  2. Please mail the completed request form, documents confirming that you are a representative in the case of request by a representative, and money order for the fee (only in the case of notification of the purpose of use and request for disclosure) to the above personal information inquiry Desk.
  3. After receiving the above request form, we will inquire about two items of personal information registered with us (e.g., telephone number and date of birth, etc.) to confirm the identity of the individual.
  4. As a general rule, we will reply to you in writing (sealed mail).

3)If the request is made by a representative, documents confirming the identity of the representative

If the person requesting disclosure, etc., is a representative, please enclose documents certifying that you are the representative and representative’s own identity.
The domicile information included in each material should be up to the prefecture, and the information after that should be blacked out.
